feat(hooks): Claude Code / Codex compatible hooks run in the kernel

The kernel runs the hooks: albedo_hooks.py reads $ALBEDO_HOME/hooks.json, runs command hooks and Python callbacks (hooks.register), and the loop asks it through a hidden cell for UserPromptSubmit, PreToolUse, PostToolUse and Stop. run() and files.edit/write are gated in the kernel too. Off by default; enable per session in /extensions.
